Sure, but this happens very rarely, so the chance this happened just by coincidence is very small. I've been a regular customer at a few places, but on the rare occasions I got a free lunch they always mentioned it was because I was a regular customer. It never happened in a context where the restaurant didn't disclose the reason. The restaurant of course have every reason to let me know they were the one's that offered me a free lunch, because then I will think well of them and continue to show up. There is no point in doing this without telling the reason. In the case of the OP, it's most likely that the guy suspected she would connect it to him, which is why he did it.
Also, my daughter happened upon a real strange lady just before Christmas that offered to buy her a pair of shoes. She let her chose a pair, passed through the cashier and paid, and outside handed them over to daughter. However, this was not anonymous either, rather she probably wanted to do a good deed, and she certainly announced it was her paying for it.
