It’s an insult. When someone says that, they are probably saying this to insult you, rather than actually thinking (or knowing) that you “aren’t much to look at.”

"She's not much to look at" is either a nice way of saying "She's kinda plain-looking" or a very nice way of saying "She's ugly".

I would say "She has a great personality" instead.

I’ve had terrible crushes on women with great personalities and somewhat “ugly” looks.

I’ve had crushes on women who some people say are “not much to look at.”

Sounds like something some schmuck would say in a bar.

No, a schmuck would use the contraction "F'ugly" or some variation thereof.

Someone who is torn between telling the truth and feeling guilty about it would at least try to say something nice.

However, using a phrase like "For a fat guy, he doesn't sweat much" won't win any new friends.

Don't ask me how I know this.
