Negotiating relationship challenges caused by autism

I was diagnosed with mild autism at age 7. I have only been in two real relationships my entire life and the current one started three years ago. I have grown a lot through this relationship but more and more I am noticing when my autism causes strife between my partner and I.

I have trouble sensing if the other person in the conversation understands what I am saying so I tend to over explain myself. This annoys my partner but I do it with everyone not just him. I also think differently than him and I guess it comes off as me opposing his viewpoint too often.

I know these aren't big issues mostly little annoyances but I think they are also impacting my work relationships and would like to know if anyone else has struggled with similar issues. If you have struggled with similar issues how did you work to overcome it? I currently try to meditate 1x a day and that has helped me become more mindful of what I say, and how I say it. But I still can be very harsh without realizing I am being harsh..
