In most of Europe, kissing a woman's hand is quite common. It's more of an honor gesture than a romantic gesture. Most commonly, it's done in romantic situation, but also when giving her an award, especially for academic or performance achievements. It's not so common in business settings. Now, the custom of kissing a woman's hand is becoming less popular, but it's still recognized.
In the US, kissing a woman's hand is far less common, although people familiar with European culture will understand its meaning. Generally, this gesture fits in well in classical art, theatre, high culture, and Renaissance fair settings. In many other situations, especially among younger people, it may seem out of place and over the top.
