I've been temporarily banned once on a Pokemon RPG site.
But it wasn't my fault or anything, because everyone got banned there because someone who was an Admin got their feelings hurt, and decided to invade the site as an act of revenge.
He got permanently banned by the owner eventually, though.

I was banned from a music fan forum for constantly arguing with the mods and admins. Myself and a group of other people had taken umbrage with the way the community was being run, and there was an ongoing drama which resulted in half the forum being shut down.
