I was wondering if there's any Assistive technology out there to help people with AS get their thoughts out into the real world/to other people.

I have always had a problem being able to communicate or show an idea I see visually I've tried drawing, painting, writing, singing, mind mapping etc with no results.

Anyone found anything out there which can help perhaps read thoughts and help you communicate those ideas to others?

What you're looking for is a BCI setup. They're still so new that there's hardly any use for them except in very special areas, like automating robotic legs for paraplegics.

(I'm working on a general solution for translating thoughts into speech patterns & actions for us aspies, but it's taking me a long time. Also, check out MIT's Media Lab, since they're working on similar things.)
