Those of you with a diagnosis did you get any paperwork?

For those of you with a diagnosis did you ever get any paper or anything tangible that says what your diagnosis is? Or were you simply told verbally.

Yes I got a five page report sent to me through the post and so did my doctor and my psychiatrist.

It was really helpful to me as well.

I have paperwork about my evaluation from six grade and it's a few pages long. At the end it said ASD, mild. then I have the follow up report and it said Asperger's, OCD, depression, and anxiety. I am not sure what happened to my official paper. My mom used to have more of my childhood medical records and half of it got lost somewhere. My mom said she isn't sure what she did with the rest so the ones I have are all the ones she has given me for work and it included my old IEPs.

Daughter: NT, no diagnoses. Possibly OCD. Is very private about herself.

To state the obvious, an official diagnosis means documentation. That is, printed manuscript of the Dx including the name/title of diagnostician, date, your name, etc.

Any diagnostic is also a legal document. So is a marriage certificate, deed, etc. No such thing as a 'verbal agreement' in any diagnostic.

For instance, should you ask for, say, an accommodation at your university (or whatever), diagnostic paperwork is required.

I'm waiting for mine. They said it would take some time to come through.

The guy said that him and the speech therapist only work the same hours once a week, and because the speech therapist was off on holiday for a couple of weeks, I'd have to wait until they see each other again to write this report up.

I'm also waiting on a referral to psychiatrist for ADHD medication. Y TAKE SO LONG NHS :wall:

Yes, I don't know how much of it related to me and my diagnosis or how much of it was documents offering advice and information. I would love to know my self but i don;t know where it is, my mother says she has not thrown it out but every time i ask about it she fails to produce it.

She probably just misplaced it.

Yes.

It's even on fancy Department of Defense stationary.

Additionally, it's on my medical records.

Nope.

Just verbal from the diagnosticians themselves.

Sure, they had to sign off on disability thingies for the government, but they never gave me any fancy paperwork themselves.

I have 3 diagnostic reports,
from first assessment,
from second assessment,
from disability assessment.
The disability assessment has another scale about how the disability affects everyday life, it's scale is about how the disorder affects your life going from 1 to 5 (I guess 5, need to look it up again).

I did get a report for my educational needs when I went to college.

That was quite extensive. I didn't even bother to read it.

I got a 2-page summary statement from the Psychologist who diagnosed me, but I'm fairly certain that what he put in my permanent file was probably a much more detailed document.

I've considered framing it, but I suppose some people might find that a bit on the bizarre side. :?

Often they will not give you the report unless you ask for. It is your right to have it, so ask for it.
