One looked at my activities of daily living and the other all the jobs I worked in the 15 years before becoming disabled. I emphasized in the activities of daily living problems with keeping my house clean, problems with socializing, conflicts with other people, problems with motivation, fear of relapse of psychosis, my recent regression, etc.

For the jobs, I went into all 14 jobs I worked (even though there was room for only 8) and showed that most jobs I couldn't do very well.

Now it's Labor Day coming up, so delivery will probably take a few days. I guess I should just sit back and wait.

My therapist is submitting the behavioral health clinician's statement for my short-term disability on Tuesday, and we'll see what goes on with that, too.

I wonder if I can ever return to work full-time.

I forgot to put that I had been neglecting my dental hygiene. I called and left a message and gave a reference for my dentist about this.

Left another message distinguishing my good days and bad days and how erratic they are.

Dang! I feel so bad leaving this important information off the form!

Maybe I should fill out another copy of the form.

I am adding to Google Calendar to call that number during business hours to make sure they understand that aspect of my case.

FYI caseworkers tend not to answer calls because they are constantly making them, so you will probably find, once you are assigned to a specific caseworker, that you always have to leave a message and wait for a callback. They do call back, but it can take a day. Leaving them too many messages before getting a callback just makes it harder for them to call you because they are listening to all your messages instead of talking to you. :)

Sounds like you did fine. A lot of people realize they left something off. Don't fill out the paperwork again. That will just be more for them to weed through.
