Classroom Accommodations Paperwork

I am having a very hard time filling out my students with disabilities paperwork. This is my second year in college and know from my experience last year that I should request them. Even though my grades were good, my mental health (and physical health) suffered. Any one have any experience with filling out these forms? They need me to tell them what kind of accommodations I need and why. How my disability (I was perhaps misdiagnosed as ADHD as a child) affects my education, and what accommodations I received in High School, but I don't know? In grade school, my teachers would just work with me on whatever I needed. I know sitting up front is something I should ask for, as well as extra test time. Would it be reasonable to request extra time on assignments? Or is that unfair to other students? I zone out in class quite a bit as well, not sure if this can help with that. I would like advice, please :)

Ask for department help at your college.

That's the first thing that popped into my brain.
Could be counseling / guidance or an offshoot office.

Nor unfair at all. Back it up with other things. Do all you can to get all that you can.
