If you really know your expressions, you would say they are all fake, because they ARE. If it were a "true" smile, they wouldn't return to a frown so quickly.
Those were my thoughts too.
Nevertheless, I took the test and got 16 out of 20, which was pleasantly surprising. I did guess on a lot of them, though, so it may have just been luck. I mainly judged it on how quickly their facial muscles relaxed.
