If your job is at risk, then see your consultant, explain the situation, then tell your manager you have just had it confirmed hence didn't mention it before.

Eye contact probs where part of what led me to develop a social phobia that was quite hard to overcome. Still causes probs now (eye contact not phobia) but usually I just make sure I'm busy so talk and work hence looking at work and glancing at colleagues.. Otherwise it is very awkward, esp staff me...

Check out some CBT self help, start with a web search on NATS (Negative Automatic Thoughts),
this helped me but obviously do not know you so cant say it is appropriate but it is interesting reading also : )
